How to Apply

1. Log on to Final Forms. https://nocacademy-oh.finalforms.com/
2. Click on ‘parent’ and create an account or log in.
3. Click the ‘+ add student’ box and enter your student’s info.
4. Click the box that says ‘Click to Switch to 2024-2025 Registration’.
5. Select your student’s name. Complete and sign all forms on the left menu until all tabs are green.
6. Click the file folder icon to upload all copies of the required enrollment documents (Birth Certificate, Parent ID, Vaccine Record, Grade Card, etc.)

NOCA’s enrollment office will contact you if your application is not complete or if your student is offered a placement after our lottery drawing on February 29, 2024. 

Students not offered a seat in the lottery will be placed on a waitlist in the event a seat should become available.
