HOW TO APPLY

APPLICATIONS FOR THE FALL 2023 LOTTERY ARE DUE JANUARY 31, 2023.  

 

  1. Log on to Final Forms. https://nocacademy-oh.finalforms.com/
  2. Click on ‘parent’ and create an account or log in.
  3. Click the ‘+ add student’ box and enter your student’s info.
  4. Click the box that says ‘Click to Switch to 2023-2024 Registration’.
  5. Select your student’s name. Complete and sign all forms on the left menu until all tabs are green.
  6. Click the file folder icon to upload all copies of the required enrollment documents (Birth Certificate, Parent ID, Vaccine Record, etc.)

NOCA’s enrollment office will contact you if your application is not complete or if your student is offered a placement after our lottery drawing on February 16. 

Students not offered a seat in the lottery will be placed on a waitlist in the event a seat should become available.
